You will be part of a multi-disciplinary team whose main function is to assess the aged care needs of frail older people and assist them to gain access to the most appropriate types of services and care, including approval for Commonwealth subsidised aged care services. |

Your new Role:
We are seeking multiple Home Support Assessors who are passionate about making a difference in the lives of others and who love working with the community. As a part of our supportive multidisciplinary team that values inclusion and diversity you will provide non-clinical home-based face to face assessments of clients referred via the My Aged Care Gateway using the Integrated Assessment Tool. Under the Single Aged Care Assessment System, you will work with clients and their representative/s to establish goals specific to the client's needs and develop appropriate strategies and actions, including referrals to aged care and non-aged care services.

Highly desirable:
- Previous experience with Regional Assessment Service
- Community or residential aged care experience
