Nurse Practitioner - Mental Health and Intellectual/ Developmental Disability Team

The Intellectual/Developmental Mental Health Disability Team (IDDMHT) are looking for a Nurse Practitioner to join their team. IDDMHT provides specialised clinical consultation support within Metro South Addiction and Mental Health Services (MSAMHS). The service provides a comprehensive response to mental health services supporting young people and adults with complex neurodevelopmental conditions. The team supports the primary treating team by providing focused assessment and management advice where required, with the aim of preventing the development of chronic issues and reducing avoidable admissions and presentations.

About the Role
- Work collaboratively with the health care team and contribute to enhanced patient outcomes by actively leading and supporting existing and emerging nurse-led models of care whilst maintaining flexibility and responsiveness to evolving patient and service needs.
- Demonstrate safe, quality, cost effective care and practice within the Nurse Practitioner's scope of clinical practice, that is consistent with contemporary standards and evidence-based practice.
- Provide coordinated consumer-centred care, creating partnerships, improving consumer outcomes and facilitating systems improvement.
- Demonstrate evidence-based practice, conduct research activities and quality initiatives to improve and inform practice change.
