The Diabetes and Endocrine Unit are looking for a Clinical Nurse Consultant to join their team. The Diabetes Street Hub is a partnership between the Metro South Hospital and Health Service (MSHHS), Micah Projects and the Inclusive Health and Wellness Hub to foster relevant expertise and collaboration between community organisations and networks, primary care, and hospital-based providers.

About the Role

• Demonstrate advanced application of clinical and professional leadership skill and specialist knowledge to achieve integrated patient care and reduce fragmentation in the patient journey.
• Responsible for the implementation of a quality and risk management framework to improve quality of care and work environment.
• Provide patient and staff education, leadership within their expertise and act as a clinical resource, mentor, role model, and technical expert within the health care team environment.
• Work closely with research partners to identify opportunities for research to inform practice change and quality improvements in clinical practice.
