Community and Oral Health, Rehab@Homeare looking for a Personal Care Worker to join their team. The primary purpose of the position is to provide domestic assistance, personal care, transport and respite care to Rehab@Home clients across Metro South Hospital and Health Service. The Personal Care Worker will work collaboratively with the client’s Key Clinician and other members of the multidisciplinary team in the provision of community care services to promote, maintain and improve the health and well-being of clients.

About the Role
- To provide domestic assistance, social support, respite care, medication prompting, personal care and transport services as part of an individual client's care plan including relevant client manual handling tasks as required and appropriate.
- To report any observed changes or relevant issues regarding the client's health or domestic circumstances, to the treating clinician.
- To accurately document all duties performed for the clients of the Rehab@Home Service.
- To utilise effective communication with clients, carers and members of the multidisciplinary team, including government and non-government community agencies.
