Your new role:
As a Clinical Nurse within the Heart Failure Service at Logan Hospital, you will provide advanced clinical expertise, leadership, and patient-centred care to support individuals with complex health needs. This role is pivotal in ensuring safe, high-quality outcomes and continuity of care from hospital to home.
Key responsibilities include:
- Delivering specialist nursing care, education, and support to patients with heart failure across hospital, clinic, home, and telehealth settings.
- Leading and coordinating care within a multidisciplinary team, ensuring best practice and adherence to clinical standards.
- Supporting the development of staff through mentoring, education, and role modelling.
- Contributing to quality improvement, research, and innovation to enhance service delivery and patient outcomes.
About you:
We are seeking an experienced and compassionate Clinical Nurse who demonstrates leadership, advanced clinical knowledge, and a commitment to person-centred care.
To succeed in this role, you will have:
- Demonstrated expertise in cardiac/heart failure nursing, including assessment, care planning, and evaluation in complex clinical situations.
- Strong communication, problem-solving, and critical thinking skills, with the ability to lead teams and influence positive change.
- A commitment to continuous learning, professional development, and supporting the education of others within the healthcare team.

Logan Hospital is the major health centre for one of the fastest growing areas in Queensland. We offer a range of services, including medical, rehabilitation, maternity, surgical and paediatric care.
