Your new role:
We are seeking a dedicated and experienced Clinical Nurse to join the Central Referral Hub within the Primary Care Partnerships Unit at Metro South Health. In this pivotal role, you will provide advanced clinical expertise, leadership, and support in managing outpatient referrals and ensuring high-quality patient outcomes.
Provide advanced clinical assessment, planning, and coordination of outpatient referrals for patients with complex care needs.
Work autonomously while supporting and mentoring the healthcare team, ensuring adherence to clinical standards and referral criteria.
Lead and contribute to clinical practice, education, research, and management activities, including quality improvement and portfolio responsibilities.
Support the achievement of team and individual Key Performance Indicators aligned with Central Referral Hub objectives.
About you:
You are an experienced nursing professional with strong clinical, leadership, and problem-solving skills who is passionate about improving patient access and outcomes. You thrive in collaborative team environments and bring a proactive, professional approach to your practice.
Current registration with the Nursing and Midwifery Board of Australia (NMBA) and a relevant tertiary nursing qualification.
Demonstrated advanced clinical knowledge, assessment, and decision-making skills, particularly in referral management and outpatient services.
Proven ability to lead, mentor, and support colleagues while contributing to education, research, and practice improvements.
Strong interpersonal and communication skills, with the ability to build effective relationships across multidisciplinary teams and manage change.
