The Older Adult Mental Health Service is looking for a Project Lead - Mental Health Hospital in the Home in Residential Aged Care to join their team. The Older Adults Mental Health Service is a multidisciplinary Service across inpatient and three community sites that provide specialist mental health services in partnership with the many primary and specialist services working towards older adults’ mental health and wellbeing.

About the Role
- To plan, develop, implement, and evaluate older adult mental health service planning and project initiatives relating to the statewide ‘Exploring Mental Health Hospital in The Home Models in Residential Aged Care' project at the direction of the Clinical Director, Older Adult Mental Health Services and in collaboration with the Mental health Alcohol and Other Drugs Strategy and Planning Branch, Clinical Planning and Service Strategy Division.
- To establish and maintain effective partnerships between stakeholders across the hospital, community and the primary health care sector, so that initiatives and projects are effectively enabled.
- To demonstrate improved outcomes for patients through agreed strategic older adult mental health initiatives.
