Responsibilities Include:
As an Intensive Education Case Manager, you will:
- Work collaboratively in a multi-disciplinary team to deliver intensive case management, provide transition supports as well as manage education, intervention and prevention initiatives relating to students who are at risk of disengagement, already disengaged or at risk of becoming involved with the youth justice system.
- Provide intensive case management that may require educational and behaviour intervention.
- Establish and sustain effective partnerships with stakeholders, government and non-government organisations including families, education providers, community organisations and other key stakeholders.
- Facilitate and lead the coordination of individual student plans and transition plans, using an integrated approach that consider a students' strengths and presenting concerns.
- Implement best practice, culturally appropriate case management for all students to improve learning and engagement outcomes.
Highly desirable skills and knowledge:
Whilst there are no mandatory qualifications related to this role knowledge and experience of the following are desirable:
- Understanding the impact of disability on individual functioning within an education context and evidence-based practices to support reasonable adjustments.
- Experience working with First Nations communities, families and students and students from culturally diverse backgrounds.
- Experience working with young people who demonstrate a range of support needs.
