About the role
The purpose of this role is to contribute to patient care by undertaking support tasks delegated under the direct or indirect supervision of Social Workers within the Specialist Assessment Intervention Liaison (SALI) team.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Undertake duties under the supervision of SALI team social workers to ensure an efficient and effective standard of service to patients/carers and their families
- Assist social workers by providing clinical support including the management of casework, counselling appointments, management of forensic room and other delegated activities
- Act as first point of contact for patients and external service providers (both government and nongovernment organisations) to ensure sensitive and professional communication and a positive service approach
- Confidently communicate with staff, patients/carers and family members to ensure high standards of service provision
- Maintain relevant clinical documentation and collection of statistics by ensuring accurate and timely data entry while complying with organisational policies and guidelines
- Contribute to patient records according to organisational guidelines and legal requirements.
- Maintain databases, resources and information relevant to the role.
- Participate in and attend departmental meetings, relevant professional development activities, and mandatory education and quality activities.
- Willingness to undertake social work assistant duties in other teams within the social work department if required.
- Undertake quality improvement activities under the guidance of SALI social workers
- Adhere to policies, procedures and guidelines relevant to the role, with particular attention to professional communication and liaison between social workers, patient, family and carers
