About the role
Temporary full time opportunity to contribute to the RBWH Pharmacy Patient Flow Team, play a fundamental role in integrating Collaborative Pharmacist Medication Prescribing (CPMP) into clinical practice and to facilitate timely medicines management on discharge. This is the first in Queensland dedicated team for CPMP and requires experienced clinical pharmacists to effectively deliver this expanded practice to optimise patient flow. The role applies high level clinical knowledge to the provision of CPMP as part of the multidisciplinary team, supports the delivery of a safe and effective Patient Flow service and contributes to the ongoing training and development of the pharmacy team.
This role works an 8-day fortnight across a 7 day roster, including extended hours.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- As a general specialist, demonstrate and deliver a high level of knowledge in clinical pharmacy and apply professional judgement to problems of significant scope and complexity, incorporating safe transition of care across different sectors.
- Undertaking timely collaborative pharmacist medication prescribing including providing assistance to and influencing as appropriate the action of other health professionals within this plan.
- Facilitate the coordination of discharge medication management including prescribing, supply and provision of medicines information
- Exercise independent professional judgement in resolving problems, with more complex clinical decisions and problem solving made with limited professional supervision and guidance.
- Exceptional communication skills to be able to liaise effectively with inpatient multidisciplinary teams on the opportunities for medication optimisation, and the plan for patient's medication as an inpatient, and as they cross healthcare settings.
