About us:

The Central Queensland Hospital and Health Service (CQHHS) Nurse and Midwife Navigator service provides a centralised CQ-wide model of delivery, with Nurse Navigator positions operating from community care settings, based at Gladstone, Biloela (Banana), Emerald (Central Highlands) and Rockhampton (Capricorn Coast, Mount Morgan, Woorabinda).

About the role:

The Generalist Nurse Navigator employs advanced clinical and professional practice capabilities to deliver a positive consumer centred health experience across the lifespan for consumers with complex and chronic health conditions.

The four pillars that underpin the Nurse Navigator role coordination of consumer centred care; Creating partnerships; Improving consumer outcomes and Facilitate systems improvement. Working within the four pillars, this role aims to facilitate a safe and efficient consumer journey, through the reduction of service fragmentation, educating and empowering individuals and coordinating consumer care.

About you:

  • Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA) and a current annual practising certificate are mandatory for this position.
  • While not mandatory, a relevant post graduate qualification, or working towards would be well regarded.

What we offer:

Total monthly remuneration value of up to $16,684 p.m.is comprised of:

  • An hourly rate between $66.85 - $72.58 p.h.
  • 12.75% employer superannuation contribution
  • 17.5% leave loading
