The Clinical Nurse Consultant, Social Worker, Occupational Therapist or Physiotherapist Senior, Aged Care Assessment is a senior leadership role responsible for guiding and training clinicians in aged care assessments. They conduct assessments in hospitals or community settings, following government guidelines and the Aged Care Act 1997. The practitioner develops comprehensive care plans centred on clients' health needs, emphasising wellness and reablement.

About us:

Aged Care Assessment is a multidisciplinary team that assesses frail clients aged 65+ (50+ for Indigenous clients) to determine their care needs for residential, respite, community care, and rehabilitation programs, aiming to facilitate access to appropriate services through comprehensive, client-centered evaluations involving clients, carers, and providers across Central Queensland.

About the role:

The Aged Care Assessment (ACA) Senior provides leadership, training, and support to clinicians, conducting assessments in hospital or community settings per Commonwealth guidelines and the Aged Care Act 1997, with a focus on social work principles, comprehensive care planning, and ensuring quality, evidence-based practice through effective communication and collaboration within the public health system.

About you:

Hold qualifications and registration with the appropriate registration authority to practice in any of the following:

    • Occupational Therapy
    • Physiotherapy
    • Social Work
    • Registered Nurse
    • Previous Aged Care Assessment experience would be an advantage.

What we offer:

Total remuneration value of up to $164,212 p.a. is comprised of:

  • An hourly rate between $66.85 - $72.58 p.h.
  • 12.75% employer superannuation contribution
  • 17.5% leave loading
