The Aboriginal and Torres Strait Islander Advanced Health Worker role functions within the Preventative Health Pathways program, which is a multidisciplinary team that promotes integrated service provision across the continuum of primary health care, consistent with principles of client self-management and Hospital Avoidance. The Aboriginal and Torres Strait Islander Health Directorate works across all facilities and provides high quality care and support to Aboriginal and Torres Strait Islander patients and their families, as well as cultural education and training for SCHHS staff.

The Aboriginal and Torres Strait Islander Health Directorate works across all facilities and provides high quality care and support to Aboriginal and Torres Strait Islander patients and their families, as well as cultural education and training for SCHHS staff.
Sunshine Coast Health is in southeast Queensland and extends through the coastal and hinterland areas from Caloundra in the south and Gympie in the north.

Key Responsibilities include:
- Deliver a range of evidence-based interventions in a culturally safe, clinically competent, professional and effective manner including the provision of co-case management, care coordination, intake assessments, cultural aspects of assessment and treatment plans, direct therapeutic interventions, referrals and cultural support.
- Undertake health programs and community care duties that may include but are not limited to: Women's and Men's health, antenatal, child and youth health, older person's health care, nutrition, substance abuse, sexual abuse, mental health, clinics, health promotion and education.
- Ability to manage own work with limited supervision through the use of planning and time management showing initiative and positive attitude.
- Demonstrate a practical understanding of Aboriginal and Torres Strait Islander inequities, and health issues to deliver informed, evidence based culturally appropriate care.
Remuneration value up to $114 335 p.a., comprising salary between $75 090 - $100 227 p.a., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HWF3).
