An exciting opportunity exists for a Pharmacist Advanced with the Specialist Palliative Rural Telehealth Service (SPaRTa) to provide comprehensive and compassionate care to address specialist palliative care needs for consumers across Sunshine Coast Health as well as leading a number of state wide services and projects. A number of services are provided including community, inpatient, outpatient, consultation liaison and telehealth services.
Sunshine Coast Health is in southeast Queensland and extends through the coastal and hinterland areas from Caloundra in the south and Gympie in the north.

Key Responsibilities include:
- Demonstrated ability to deliver an advanced level of clinical skill in one or more areas of a complex nature with the ability to independently apply specialist and novel judgement to meet the needs of patients across the continuum of care.
- Exercise independent clinical judgement in obtaining accurate patient medication history, comprehensive medication reviews, patient education and counselling, formulation of pharmaceutical care plans utilising high-level knowledge and clinical skills, including the provision of advice to medical, nursing, pharmacy staff and other health care professionals via in-person, telephone, or virtual care mediums.
- Provide high-level clinical support, advice, and education to staff within the multidisciplinary team (MDT) regarding the management of clinically complex palliative care patients. Participate in education delivery within SPaRTa regions, across hubs and within the SCHHS.
- Possess advanced clinical leadership abilities and facilitate the integration of services across specialties within a clinical area to coordinate the delivery of effective Palliative Care services across the SCHHS.
Remuneration value up to $172 615 p.a., comprising salary between $144 991 - $151 289 p.a., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HP5).
