Clinical Nurse - Community Connect

The Clinical Nurse (CN) role within Community Connect leads and delivers person centred nursing care at an advanced level based on the Nursing and Midwifery Board of Australia Registered Nurse Standards for Practice to help facilitate a safe transition from hospital to the community, particularly for people who are deemed ‘at-risk’. The CN is expected to practice autonomously and provide leadership in clinical decision making. They have the responsibility for professional leadership in the absence of the Nurse Grade 7 and an opportunity to work collaboratively to lead practice changes and clinical innovation.

The CN is expected to practice autonomously and provide leadership in clinical decision making. They have the responsibility for professional leadership in the absence of the Nurse Grade 7 and an opportunity to work collaboratively to lead practice changes and clinical innovation.
Community Connect provides a referral based, person centred assessment in both the acute and/or community setting. Referrals may be passive through criteria or initiated by health care providers. The service assists with complex discharge within the acute setting and hospital avoidance and education in the community setting. This service leverages from planning within an interprofessional, multi-disciplinary framework with the aim to provide optimal patient outcomes and optimised patient flow.
Sunshine Coast Health is in southeast Queensland and extends through the coastal and hinterland areas from Caloundra in the south and Gympie in the north.

Key Responsibilities include:
- Optimise continued recovery at home by enabling access to required community-based services; initiate referrals as required.
- Participate in multi-disciplinary discharge planning and refer as required.
- Provide education and tailored support for identified needs to support ongoing needs / facilitate engagement with services.
- Undertake community-based comprehensive assessments as allocated through the Community Home Support Program.
Remuneration based pro rata on equivalent full time value up to $131 900 p.a., comprising salary rates: $54.44 - $58.30 p.h. employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (Nurse Grade 6 (1). (Multiple Fixed Term Temporary Part-Time, 40 hrs p.f. positions available for up to 3 months).
