We’re excited to welcome an Internal Residential Aged Care Placement Clinical Assistant/Allied Health Assistant to our Social Work team. The Aged Care Placement Assistant role presents an opportunity to improve patient outcomes and enhance hospital efficiencies by streamlining the discharge of patients into residential aged care homes (RACHs) and/or home. The role focusses on assisting with building and maintaining strong relationships with local RACHs, collaborating with multidisciplinary teams and helping patients transition smoothly to appropriate care settings as described in the RACH discharge guideline. This role will further support the Hospital and Health Service (HHS) to facilitate timely identification of residential aged care home placements suitable to the needs of patients admitted in acute, sub-acute and interim care beds and transition patients back to the community.

The Aged Care Placement Assistant role presents an opportunity to improve patient outcomes and enhance hospital efficiencies by streamlining the discharge of patients into residential aged care homes (RACHs) and/or home. The role focusses on assisting with building and maintaining strong relationships with local RACHs, collaborating with multidisciplinary teams and helping patients transition smoothly to appropriate care settings as described in the RACH discharge guideline. This role will further support the Hospital and Health Service (HHS) to facilitate timely identification of residential aged care home placements suitable to the needs of patients admitted in acute, sub-acute and interim care beds and transition patients back to the community. 

Sunshine Coast Health is in southeast Queensland and extends through the coastal and hinterland areas from Caloundra in the south and Gympie in the north.

Key Responsibilities include:

  • Assisting multidisciplinary teams with building and maintaining relationships with local RACHs.
  • Maintaining updated records of relevant contacts within local RACHs to enable relationships between the HHS and RACH.
  • Working cooperatively and effectively with patients, their families, carers, and other relevant stakeholders, as part of a multidisciplinary team, to provide high-quality, patient-centred care
  • Actioning instructions from the primary clinician responsible for the patient to confirm the patient's clinical needs and ensure RACHs presented to the patient are suitable to accommodate their needs.  

Remuneration value up to $9866 p.m., comprising salary between $2624.50 - $2759.10p.f., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (CA3) (Two Fixed Term Temporary Full-Time positions available for six months).
