About the role
Fixed term temporary full time opportunity to improve patient outcomes and enhance hospital efficiencies by improving and streamlining the discharge of patients into residential aged care homes (RACHs). This role focusses on assisting with building and maintaining strong relationships with local RACHs, collaborating with multidisciplinary teams and helping patients transition smoothly to appropriate care settings as described in the RACH discharge guideline.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Maintain updated records of relevant contacts within local RACHs to enable relationships between the HHS and RACH.
- Action instructions from the primary clinician responsible for the patient to confirm the patients clinical needs to ensure RACHs presented to the patient are suitable to accommodate their needs.
- Work with clinicians to provide information so RACHs have the information they need to support the person as they transition to being a resident.
- Support patients and families/carers to complete an assets and income form if required and confirm its lodgement.
