Your expertise will guide patient care and foster collaborative relationships across the South Westh Hospitalt and Health Service —all within an environment that seamlessly blends professional growth with the indulgence of a relaxed, rural lifestyle.
Provision of evidence-based and quality critical care of individual patients in all domains of medical practice:
- Patient interview;
- Clinical examination;
- Identification of clinical problems and health opportunities;
- Plan of overall clinical care and health promotion;
- Focus on chronic disease identification and management, team care arrangements with appropriate key performance indicators aligned with improvement of the burden of chronic disease within the community
- Maintenance of concise but comprehensive, accurate, legible records of patient status and progress and associated correspondence;
