Tell me more...The Care Co-ordination Service Centre ensures the complex health care needs of First Nations priority patient cohorts is culturally inclusive and coordinated to enable access to required care in the most appropriate location and at the right time. The CCSC promotes equity in health outcomes through streamlined logistical and care coordination support.
