Townsville Hospital and Health Service are seeking a Nurse Navigator (Identified) to join the team within Chronic Disease Service at the Townsville University Hospital on a permanent part-time basis, working 56 hrs per fortnight.

The Nurse Navigator (Identified) facilitates person-centred care, integrated care pathways through coordination of models of care and service access for patients with chronic conditions and/or complex healthcare needs.

About the work unit / facility:

The Nurse Navigator Service meets the needs of the patient and is intended to be flexible and responsive to Townsville Hospital and Health Service (HHS) needs. Navigators work across multiple specialties remaining focused on the patient and their journey. The role scope is not limited by a disease process. Functionally, this means that the Nurse Navigator care of a patient will grow and adapt with the changing needs of the patient irrespective of the traditional medical or nursing specialities that would define care in other contexts.

Competitive salary and benefits:

  • Salary between $66.85 to $72.58 per hour (NG7).
  • 4 to 6 weeks p.a. recreational leave with 14-17.5% loading.
  • Generous employer contribution to superannuation of 12.75%.

About your opportunity:

This role focuses on supporting our Aboriginal and Torres Strait Islander people with chronic conditions and complex care.  The opportunity assists to improve health care outcomes for this patient cohort with a proactive approach to streamlining their healthcare journey and engaging primary health care services.

Requirements:

  • Mandatory qualification and registration with AHPRA.
  • Identified position: Under section 25 of the Anti-Discrimination Act 1991, there is a genuine occupational requirement for the incumbent to be indigenous to the Aboriginal or Torres Strait Islander community.
  • Possession of a relevant postgraduate qualification in an appropriate field is highly desirable.
  • Possession of class C drivers' licence.
  • This is a Vaccine Preventable Diseases (VPD) risk role.
  • A probationary period may apply.
  • Pre-employment checks will be conducted.
