About the role
The service provides care to patients with diabetic conditions and specialist diabetes nursing care supporting the general function of the Outpatient Department through the application of the principles of best practice, education, leadership, and portfolio management.
The Nurse Practitioner (NP) – Paediatric Diabetes is an endorsed registered nurse who demonstrates advanced and expanded clinical skills, knowledge and high level of capacity to provide contemporary evidence based clinical care for Paediatric Diabetic patients in the Ambulatory/Outpatient setting.
The Nurse Practitioner, Paediatric Diabetes:
- is an autonomous practitioner working within a collaborative model of care.
- Provides an advanced and expanded clinical role capable of assessment, care planning and intervention.
- Provides advanced level of autonomous decision making.
- Has a credentialed scope of clinical practice for the care of patients within the Outpatients Paediatric Diabetes Service.
About you
Our ideal candidate will be someone who can demonstrate the following:
- Demonstrated contemporary advanced practice, expert clinical skills and knowledge in credentialed scope of practice and leadership capability enabling the delivery and support of high-quality patient focused
- Demonstrates the ability to apply specialised knowledge of contemporary advanced nursing practice and theory in credentialed scope of practice and to participate directly and indirectly in the delivery of clinical care.
- Demonstrates the ability to build and maintain strong relationships, open communication and a positive environment within and across the multidisciplinary team, actively contributing to the learning and development of others to a high standard.
- Demonstrates the ability to educate, inform, counsel and support patients and families with contemporary clinical information at an advanced level relevant to patient's care and understanding.
- Applies knowledge of quality improvement frameworks and associated quality processes including the integration of contemporary nursing research to all clinical activities.
- Demonstrated ability to interact professionally and maintain professional relationships with all members of the healthcare team both internally and externally to the health service.
Mandatory Requirements:
- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
- Appointment to this position requires registration with the Australian Health Practitioner Regulation Agency (Ahpra) as a Registered Nurse with Endorsement as a Nurse Practitioner and a current annual practising certificate are mandatory
