This service is dedicated to optimizing the patient journey, reducing delays in care, and ensuring timely access to the right services, in the right place, at the right time.
By focusing on the coordination of medical admissions, transfers, and discharges, this service aims to address bottlenecks in patient flow, improve bed utilization, and enhance overall operational efficiency, while maintaining high standards of patient care.

About the role
The Clinical Nurse in the Flow Improvement Service Medical (FISM) will play a pivotal role in driving timely and efficient patient flow. This position focuses on supporting the multidisciplinary team (MDT) to proactively manage discharges, reduce unnecessary delays, and ensure patients can return home or transition to the next stage of care as soon as clinically appropriate. As a leader in promoting flow, the Clinical Nurse will champion criteria-led discharge processes and foster a culture of proactive planning and collaboration across the team.
This new service is pivotal in addressing current challenges in patient flow while supporting the strategic goals of the Division of Medicine. By fostering a culture of collaboration, innovation, and efficiency, the Medical Flow Coordination Service will play a critical role in delivering exceptional patient outcomes and operational excellence.
This role is an exciting opportunity for a dynamic and proactive Clinical Nurse to make a significant impact on patient care and operational efficiency, driving positive outcomes for both patients and the organisation.
About you
Our ideal candidate will be someone who can demonstrate the following:
- Demonstrated advanced clinical knowledge of Acute Medical Nursing and in delivering expert nursing care in complex situations while providing support and direction to other team members.
- Demonstrated ability to communicate effectively with members of the healthcare team, patients, relatives and visitors and maintain a positive workplace culture through well developed consultation, liaison, negotiation and conflict resolution skills.
- Demonstrated ability to build and maintain strong relationships and a positive environment within a multidisciplinary team, actively contributing to the learning and development of others to a high standard.
- Demonstrated ability to participate in the multidisciplinary team to identify areas for improvement in work practices and outcomes and contribute to the continued improvement of policies and procedures.
- Ability to prioritise clinical and related administrative duties, including effective and efficient planning and organisation of complex caseloads with proven ability to use information and reporting systems appropriate to the position.
- Demonstrates a strong team commitment through ability to provide clinical education and support at an advanced level.
Mandatory Requirements:
- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
