About the role

The Nurse Navigator is an advanced clinical practice registered nurse who demonstrates clinical knowledge, skills and abilities at advanced levels and is able to translate these into contemporary evidence-based care to practice across the system of care. The Nurse Navigator role aims to facilitate safe and efficient care for complex patients by reducing service fragmentation, enhancing management in partnership with the family and carers through understanding of their condition and coordinating the connection to appropriate services to enhance outcomes thus smoothing the patient journey across the health landscape. This role will be the Lead Coordinator for the Nurse Navigator Service and the complex care portfolio.

About you

Our ideal candidate will be someone who can demonstrate the following:

  • Demonstrated ability to apply effective contemporary clinical knowledge and clinical management skills in the management of complex care.
  • Demonstrated ability to build and maintain strong relationships and to develop strong teams across the health landscape within a framework of open communication.
  • An ability and commitment to deliver clinical services collaboratively integrating the broader multidisciplinary health care team across the system in delivering high quality, patient centred care.
  • Demonstrated contemporary clinical nursing expertise with clear understanding of the requirement for evidence- based patient centred care.
  • Demonstrated ability to work autonomously and provide independent professional judgement and expertise in patient centred problem solving and clinical management.

Mandatory Requirements:

  • Appointment to this position requires proof of qualification and registration with Ahpra. Certified copies of the required information must be provided to the appropriate supervisor/manager, prior to the commencement of clinical duties.
  • Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
