About the role
The role of the Clinical Nurse Consultant (CNC) in Hospital in the Home (HITH) is to provide clinical expertise and consultancy through advanced clinical leadership relevant to acute care in the community. The CNC provides direct and indirect care as required and works in collaboration with the Nurse Unit Manager and wider interdisciplinary health care team supporting the clinical unit and the hospital in provision of safe quality healthcare through exemplary clinical nursing standards of practice.
About you
Our ideal candidate will be someone who can demonstrate the following:
- Demonstrated ability to apply advanced knowledge of contemporary nursing practice and participate directly and indirectly in the delivery of patient centred consultancy and clinical care with the ability to work independently with a high degree of initiative.
- Demonstrated excellence in communication and inter-personal skills across a variety of mediums and ability to build and maintain strong relationships, open communication, and a positive environment within a multidisciplinary team inclusive of external stakeholders.
- Proven ability to implement changes and develop direction in clinical practice by the utilisation of evidence-based research and best practice principles.
- Demonstrated skills in strategic and clinical service planning including an understanding of organisational behaviour, change management and cultural change processes.
- Demonstrated skills in clinical teaching, competency in the use of contemporary information systems and experience in the development of specialty education programs.
- Demonstrated ability of advanced clinical skills and judgement and engagement in developing and applying specialty clinical models of care aimed at providing care in an alternate location.
Mandatory Requirements:
- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
- Minimum five years full time equivalent post registration experience, with at least 3 years full time equivalent experience in a HITH style service or senior nursing leadership role and an extensive hospital alternative care delivery knowledge.
