About the role
Chronic condition care is delivered by a multidisciplinary team—comprising of nursing staff including Nurse Practitioners, allied health staff and administration staff and the team works collaboratively to support patients across the continuum of care, from initial diagnosis to advanced disease management and symptom management prior to progressing to palliative care.
The service focuses on key areas including Heart Failure Service, Lung Health Service, and Cardiac Rehabilitation, ensuring patients receive targeted and coordinated care. Whether supporting individuals with chronic respiratory disease, guiding recovery after cardiac events, or managing long-term heart failure. The team aims to improve health outcomes, enhance quality of life, and empower patients to take an active role in their care through education, self-management strategies, and integrated support.
The Nurse Unit Manager - Chronic Conditions Service is a registered nurse who will lead the Chronic Conditions Service programs strategically and operationally and be responsible for the reporting of the key performance indicators for the programs. The position is designated as the single point of accountability for the operations of the Chronic Conditions Service program and works within the context of the multi-disciplinary team.
About you
Our ideal candidate will be someone who can demonstrate the following:
- Advanced experience skills and knowledge in leading, patient centred models of care for complex chronic conditions across the care continuum.
- Ability to delivery contemporary and evidence-based services across community and primary care working in a collaboratively to support integration and innovation across sectors.
- Ability to operationally manage across professions and multidisciplinary team environment to ensure effective service delivery and quality outcomes.
- Proven ability in the application of highly developed interpersonal, problem solving, negotiation and communication skills, relationship development and maintenance and change management skills.
- Demonstrated management knowledge and skills specifically human resource management and advanced financial literacy within the healthcare environment.
Mandatory Requirements:
- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
