About the role
The Clinical Nurse is a registered nurse who is accountable for practice at an advanced level, working collaboratively within the unit and with other members of the health care team including consumers/carers, other health professionals, general practitioners and other agencies (including non-Government agencies). This role provides the delivery of clinical care and support specific to the Secondary Prevention for All in Need (SPAN) clinical trial within the Cardiac Rehabilitation team attached to the Chronic Conditions Services and Community Health setting.
The Secondary Prevention for All in Need (SPAN) clinical trial funded by the Heart Foundation of Australia and is comparing standard outpatient hospital cardiac rehabilitation versus a more personalised form of cardiac rehabilitation in patients after Type 1 Myocardial Infarction.
Participants in the trial are randomised 1:1 to either standard or personalised cardiac rehabilitation. The Personalised Cardiac Rehabilitation Nurse will work part-time on SPAN with the main responsibility of conducting the personalised rehabilitation program on participants randomised to that treatment arm. The nurse will provide cardiac rehabilitation services, support patient choice and flexibility in the range of services available and assist in the timing and location of services.
About you
Our ideal candidate will be someone who can demonstrate the following:
- Demonstrated advanced clinical knowledge in the area of Cardiac Rehabilitation and Chronic Disease Management delivering expert nursing care in complex situations while providing support and direction to other team members.
- Demonstrated ability to role model excellent standards of comprehensive, evidence-based nursing care and documentation to facilitate quality patient outcomes, in accordance with National Safety and Quality Health Service and professional standards.
- Demonstrated ability to communicate effectively with members of the healthcare team, patients, relatives and visitors and maintain a positive workplace culture through well developed consultation, liaison, negotiation and conflict resolution skills.
- Demonstrated ability to support and lead quality improvement and research initiatives within the work unit, contributing to the use of evidence based practice to facilitate improved patient outcomes.
- Ability to prioritise clinical and related administrative duties, including effective and efficient planning and organisation of complex caseloads with proven ability to use information and reporting systems appropriate to the position.
- Demonstrates honesty, integrity and respect for all patients, carers and staff.
Mandatory Requirements:
- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
