About the role
HITH operates a 7-day service, inclusive of weekends, public holidays. The Clinical Nurse in the HITH service is an expert in clinical care provision to provide substituted hospital care to patients within their home setting, with extensive experience in Acute and Community nursing practices. This role supports the general function of the unit through the application of the principles of best practice, education, leadership, and portfolio management.
The HITH team operates collaboratively with community-based services, residential aged care facilities, hospital inpatient teams and multidisciplinary Allied Health services to provide evidence-based care to clients who meet specific criteria within the HITH West Moreton Hospital and Health Service catchment. The HITH team provides acute care through an integrated model of care to patients requiring medical management of patients experiencing acute illness in their place of residence, who would otherwise be required to remain in hospital to receive the same care. This role is required to work varying shifts across a 7 day a week roster, including public holidays.
About you
Our ideal candidate will be someone who can demonstrate the following:
- Demonstrated advanced clinical knowledge in Acute Nursing Care delivering expert nursing care in complex situations while providing support and direction to other team members. Advanced clinical experience in care of the older person in an acute care setting will be desirable.
- Demonstrated ability to role model excellent standards of comprehensive, evidence-based nursing care and documentation to facilitate quality patient outcomes, in accordance with National Safety and Quality Health Service and professional standards.
- Demonstrated ability to communicate effectively with members of the healthcare team, patients, relatives and visitors and maintain a positive workplace culture through well-developed consultation, liaison, negotiation and conflict resolution skills.
- Demonstrates a strong team commitment through ability to provide clinical education and support at an advanced level.
- Demonstrated ability to participate in the multidisciplinary team to identify areas for improvement in work practices and outcomes and contribute to the continued improvement of policies and procedures.
- Demonstrated ability to coordinate staff and manage delegated resources including prioritising needs and supporting day-to-day changes.
Mandatory Requirements:
- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
