About the role
The purpose of the role is to provide Social Work clinical and leadership services within a hospital based multidisciplinary team in accordance with the Australian Association of Social Workers (AASW) Professional Practice Standards and Code of Ethics. This position provides psychosocial assessment and works in collaboration with the multidisciplinary team to incorporate psycho-social needs in planning care; assists in developing resources to meet patient needs; participates in patient and family orientation, education, and in-service activities to promote understanding of the psychosocial impact of chronic kidney disease.
The Renal Outpatient Social Worker's primary responsibilities are to help patients and family members adjust to the diagnosis of CKD / ESKD, help patients manage the multiple daily demands of treatment and self-care, promote quality of life, and provide support, education, and counselling at all stages of the disease. The Social Worker promotes leadership through collaborative partnerships and advocacy to help meet patient needs and support the continuum of care. All positions are required to provide 7-day availability for After Hours, On-call rostering to respond to clinical emergencies for patients who meet specified clinical criteria at Ipswich Hospital.
About you
Our ideal candidate will be someone who can demonstrate the following:
- Competence and experience in delivering Social Work clinical services in accordance with the Australian Association of Social Work (AASW) Code of Ethics and Professional Practice Standards using well developed psychosocial assessments and intervention skills and practice evidence informed clinical care, counselling, group work and advocacy in the health care system.
- Commitment and leadership for active and ongoing professional development and supervision to meet the CPD hours set by the AASW.
- Competence in professional clinical practice and adherence to defined service standards and the management of complex patient care, whilst exercising a degree of independence and professional decision making, accessing clinical practice supervision and guidance as required.
- Effective written communication skills including psychosocial assessments, correspondence with service providers and clinical reports and the provision of practice within the medico legal requirements.
Mandatory Requirements:
- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
