SWHHS operates under a Hub and Spoke Model with services managed from the hub hospitals in Charleville, St George and Roma. The spoke sites include Residential Aged Care Facilities, Multipurpose Health Services and Community Clinics.

SWHHS' mandatory onboarding requirements include:
- Current AHPRA registration;
- Vaccine Preventable Diseases – Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B;
- May require the current years influenza vaccination (during the peak season);
- National Police Certificate (aged care criminal history clearance); and
- May require a current NDIS Worker Screening Check (dependent on location).
