About the role

  • The purpose of this role is to apply Occupational Therapy skills and knowledge in the assessment and treatment of clients in the local population to promote, maintain and improve their health, functional ability and quality of life.
  • The Post Acute Care Service (PACS) provides a post-acute hospital avoidance response to people who are discharged from hospital or who present to emergency departments and are not admitted or referred by GP or QAS and do not require hospital admission. Short term multi-disciplinary interventions are provided to address immediate care needs. Services are provided in-home or clinic based. PACS operates seven days a week including public holidays with extended opening hours – 0730 hours to 1800 hours. Further extension of operating hours may occur based on client and hospital and health service need in the future.

About you

This successful applicant will carry out the following key accountabilities in accordance with the Metro North Health's values:

  • Deliver quality Occupational Therapy clinical services across a variety of caseloads in a post-acute community setting in accordance with prescribed professional and ethical standards
  • As experience builds, apply increasingly independent professional clinical judgement and problem-solving skills to more complex clinical decisions under clinical practice supervision or professional guidance of a more senior clinician.
  • Actively contribute to service and caseload management within the discipline of Occupational Therapy and the multidisciplinary Post-Acute Care team
  • Work cooperatively and effectively within the team and with patients/clients, their families, community groups, other health professionals, supervisors, and students
  • Provide student education and clinical practice supervision for less experienced practitioners, commensurate with the level of experience, as well as direction to assistant, support staff and work experience students, under guidance of senior clinical practitioners.
  • Actively participate within the discipline of Occupational Therapy and the multidisciplinary team's clinical environment to ensure Occupational Therapy services are integrated, efficient and equitable for the community serviced.
