An exciting opportunity exists for a Clinical Nurse Consultant-Homeless Health Outreach Team to lead and deliver person centred nursing care at an advanced level based within the Homeless Health Outreach Team. Homeless Health Outreach Team (HHOT) provides comprehensive assessments, care co-ordination and clinical interventions for homeless persons in the community who are experiencing mental illness. The service aims to minimise the health problems of people experiencing homelessness and contribute to the prevention and reduction of homelessness within Queensland through cross-sectoral collaboration, to, to facilitate access to the most appropriate supports. This is achieved through the provision of specialist mental health and general health care services to assist homeless people who are sleeping in public spaces, squats and other improvised dwellings.

Sunshine Coast Hospital and Health Service (SCHHS) is in southeast Queensland and extends through the coastal and hinterland areas from Caloundra in the south and Gympie in the north.

Key Responsibilities include:
- Lead and deliver expert clinical leadership in the provision and coordination of contemporary, person centred mental health care within a dynamic health environment at an advanced level based on the Nursing and Midwifery Board of Australia – Registered Nurse Standards of Practice.
- Within the Mental Health setting deliver high quality, evidence-based consumer focused case management, providing the opportunity for the consumer to complete individual recovery and relapse prevention plans, receive evidence based therapeutic interventions, education regarding mental illness and contemporary treatment options, information and referral to other specialist organisations (government and non- government) as well as education, consultation and support for families and carers. Education on the Mental Health Act 2016 is provided. The service is recovery oriented, person centred, family inclusive, collaborative and empowering; and operates on the premise that consumers can and do recover from mental illness and drug related disorders.
- Participate in Discharge planning when a person is at a stage of recovery where they have graduated to needing less intensive care and have supports in place to manage in the community.
Remuneration value up to $164 212 p.a., comprising salary between $132 562 - $143 924 p.a., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (Nurse Grade 7).
