About you:

In your new role you will:

  • Provide support services to determine the priority needs of senior Queenslanders, including the assessment of aged care requirements and a summary of assessment findings, goals and recommendations.
  • Identify clients who would benefit from a Reablement Focused Program and action referrals to the appropriate services.
  • Review Reablement Focused Programs within appropriate timeframes to assess need for extension or cessation of services.
  • Maintain communication and working relationships with multidisciplinary health teams, government agencies and external service providers.
