About Geriatric Evaluation and Management / Acute Cognitive Unit 

The Geriatric Evaluation and Management Unit (GEM) provides comprehensive care for older patients, offering multidisciplinary support for both clinical issues functional rehabilitation which also includes an Acute Cognitive Unit (ACU) dedicated to managing patients living with dementia and experiencing severe Behavioural and psychological symptoms of dementia (BPSD), providing a highly structured environment with psychogeriatric support.

About your new role

As a valued member of the GEM and ACU team, you will deliver comprehensive, person-centred, and evidence-based care within a supportive, multidisciplinary environment. Your focus will be on promoting the wellbeing of older adults by partnering closely with patients and their families to reduce the impact of hospitalisation and improve quality of life.  This includes

  • Providing advanced clinical support to the team, demonstrating, coaching and advising
  • Providing leadership in professional decision making through the application of managing older persons living with dementia, experiencing delirium or BPSD
  • Working collaboratively with peers and others to create team cohesion and a positive practice environment that achieves best practice outcomes
  • The ability to lead and work effectively within a multi-disciplinary setting

Qualifications

  • A Bachelor of Nursing or equivalent
  • Registration with the Nursing Midwifery Board of Australia
  • A Masters or higher qualification within a relevant area is highly desirable
  • Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B
  • Tuberculosis screening, to identify if the tuberculosis vaccination may be required
  • Declaration of serious discipline history must be disclosed
  • Criminal History Screening will be required for new employees (GCH will cover cost)

  • Permanent full time
  • Base salary ranging from $132 562- $143 924 per annum
