About the role

The Diabetes Service delivers nurse led diabetes education clinics and contributes to a broader diabetes multidisciplinary medical and allied health team to provide expert, quality, accountable care and education in diabetes to patients across the care continuum and in accordance with prescribed professional and ethical standards and evidence-based practice. The Clinical Nurse, Diabetes Service may work across all areas of patient service provision including outpatient and inpatient education services for people living with type 1, type 2, Gestational and Diabetes in Pregnancy including the utilisation and interpretation of diabetes technology ( CSII, CGMS).

Applicants must hold a Post Graduate Qualification in Diabetes Education and Management, and credentialing through Australian Diabetes Educators Association (or working towards credentialling).

About you

Our ideal candidate will be someone who can demonstrate the following:

  • Demonstrated advanced clinical knowledge in patient assessment, education, and management for patients (adults & children) living with diabetes, contemporary insulin systems (MDI and CSII) and skills in flash and CGMS initiation and data interpretation delivering expert nursing care in complex situations while providing support and direction to other team members.
  • Additional consideration will be given to applicants who can demonstrate experience in providing evidence-based self-management education and care activities for children and young people and their families diagnosed with diabetes (of any type including any insulin requirement).
  • Demonstrated ability to build and maintain strong relationships and a positive environment within a multidisciplinary team, actively contributing to the learning and development of others to a high standard.
  • Demonstrates a strong team commitment through ability to provide clinical education and support at an advanced level.
  • Demonstrated ability to communicate effectively with members of the healthcare team, patients, relatives and visitors and maintain a positive workplace culture through well developed consultation, liaison, negotiation and conflict resolution skills.
  • Ability to prioritise clinical and related administrative duties, including effective and efficient planning and organisation of complex caseloads with proven ability to use information and reporting systems appropriate to the position.
  • Pro-actively manages own continuous learning and development.
