What's involved – As the Clinical Nurse Consultant (Chronic Conditions) lead, you will have the chance to positively impact on the prevention and management of chronic conditions within our community. By applying your knowledge and expertise while providing leadership to nursing staff, you will ensure the delivery of high-quality, patient-centered care within specific chronic and complex conditions portfolios within the Primary Healthcare Team.
This role offers a unique opportunity to influence behavioural risk factors and enhance chronic disease models of care facilitate clinical improvement initiatives, support collaborative service objectives, and encourage the professional growth of your team. The principal objective is to reduce the risk of long-term complications, facilitate early discharge from hospital and reduce the risk of hospitalisation for patients with complex chronic disease conditions.
As a member of the Primary Health Team, the role will work collaboratively and more broadly with the team and across Central West Hospital and Health Service (CWHHS) to ensure continued focus on strategic priorities and delivery of operational imperatives.
