This role is a community-based position that supports the seamless flow and timely transition of consumers receiving care within the Continuing Care. The purpose of the role is to provide expertise, consultancy and relationship building to continuing care teams Community service line. Based in Southport, Robina and Palm Beach.

The Role

An opportunity has become available for a Social Worker - Senior (Mental Health - Transition and Flow Coordinator) to join Gold Coast Health (GCH) where the focus of our 12 000-strong team is on providing world-class health care to people living in the Gold Coast, and northern New South Wales region of Australia.

You will

  • Deliver high level clinical services in accordance with Social Work professional standards and code of ethics and legislation
  • Provide high level independent mental health services across community settings
  • Participate in lifelong learning, professional development, and research activities
  • Utilise high level knowledge of systems and communication skills to develop effective professional relationships to work collaboratively with all stakeholders 

About you

To be successful in this position you will require:

  • A tertiary degree in Social Work
  • Eligible for membership to the Australian Association of Social Workers (AASW)
  • Driver licence
  • Blue card / Working with children check (GCH will cover cost)
  • Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B
  • Tuberculosis screening, to identify if the tuberculosis vaccination may be required
  • Declaration of serious discipline history must be disclosed
  • Criminal History Screening will be required for new employees (GCH will cover cost)

Salary

  • Temporary full time (12 months, with the possibility of extension)
  • Base salary ranging from $128 169 - $137 918 per annum
