Logan Adult Mental Health Service are looking for a Multi-disciplinary Team Leader to join their Mobile Intensive Rehabilitation Team. This team provides intensive psycho-social rehabilitation services to consumers with severe and complex mental health needs that focuses on assisting the consumer to meet their individual recovery goals, with a focus on functional improvement across domains, to prepare the consumer for their care to be eventually transitioned to the private sector. The role provides operational management and strategic leadership for an interprofessional team of health professionals within the Logan Mobile Intensive Team.

About the Role

• Accountable for delivery of contemporary nursing practice, patient outcomes and the achievement of associated service performance targets/indicators within the Mobile Intensive Rehabilitation Team.
• Plan and lead the Mobile Intensive Rehabilitation Team human, material and financial resource management strategies and processes, regularly evaluating to ensure effectiveness.
• Work collaboratively with peers and others to create team cohesion and a positive practice environment that achieves best practice outcomes for patients and/or service.
• Coordinate mentoring, preceptorship, role modelling and promotes the value of nursing within the healthcare team in Logan MSAMHS.
