Multidisciplinary Team Leader (Pathways)

The Office of the Executive Director of Allied Health are looking for a Multidisciplinary Team Leader (Pathways) to join their team. The Pathways Team is an interprofessional team of Allied Health Professionals with the goal to address the top three barriers to long stay patient discharges identified in the long stay census: access to Residential Aged Care beds, along with barriers pertaining to NDIS and legal matters. The team of Social Workers, Psychologists, Occupational Therapists, Physiotherapists, Clinical Assistants and Onsite Public Guardians work collectively across Metro South Health Hospitals to facilitate discharge of complex long stay patients.

About the Role
- Lead and manage Pathways Team services, including services of a complex nature.
- Facilitate evidence-based practice of the interprofessional Pathways team members to ensure efficiency and effectiveness of service provision and discharge planning, through implementation and facilitation of quality initiatives, literature reviews, attendance at professional development activities and involvement in research and education.
- Lead and support the delivery of clinical services in accordance with accepted professional practices, standards and principles including the legal and ethical requirements with a high degree of independence.
- Apply advanced level independent professional judgment regarding the co-ordination and treatment planning of complex patients, as it relates to case management, discharge planning and service transition.
- This role will incorporate a clinical consultancy role for Long Stay patients with high complexity.
