Metro South Addiction and Mental Health Services are looking for a Senior Rehabilitation Therapy Aide to join their team. This team provide inpatient, hospital based and community mental health services in addition to alcohol and drug service to all age groups at a number of campuses across Metro South Health. MSAMHS is committed to the provision of services that are patient centred and recovery focussed. This position provides a comprehensive psychosocial intervention and facilitate community linkages to support mental health consumers to access a range of services to promote recovery.

About the Role

• Provide a comprehensive psychosocial assessment and subsequent formulation of support plan.
• Liaison with identified clinical treating teams where appropriate.
• Identify and maintain a network of referral and professional supports.
• Coordinate referrals/applications to housing providers including Government NGO's and private providers.
• Work as a member of a multidisciplinary team and attend team meetings for purpose of information sharing and education.
