Your opportunity

The South West Palliative Care Service provides comprehensive and compassionate care to address palliative care needs for clients and their families/carers living in South West communities.

The Service collaborates with many other integral organisations in the complex care delivery to clients, utilises and encourages innovation, quality and research to obtain optimal person-centred goals of care.

This role is required to work flexibly and will be required to travel to consult and engage with facility multidisciplinary teams, service partners, clients and their families/carers.

The Nurse Practitioner Palliative Care will:

  • Provide an advanced scope of palliative clinical practice including expanded skills and knowledge in advanced assessment, care planning and implementation, medication management, diagnoses, and evaluation of palliative nursing care.
  • Plan, provide, support and coordinate specialist palliative care to clients and their carers across the continuum of care to reduce hospital admissions and decrease inpatient length of stay, including pre-emptive planning to reduce crisis management.
  • Provide professional/clinical consultancy, collaboration and system expertise to outpatients, carers, community, inpatients and other health service providers as required.
  • Engage with acute hospital services, when appropriate, to facilitate direct admissions to and from inpatient settings across the continuum of care.
  • Provide clinical leadership, mentorship and support for nurse practitioner candidates, nursing staff within the service and other relevant staff.

Demonstrate transformational leadership and team commitment through open communication and active participation in development and achievement of service goals, including creating alignment across services within South West Hospital and Health Service.

Critically appraise and integrate relevant research findings into decision making about health care management, patient/client/carer interventions and education of nurses and other professionals.
