About the role

The purpose of this role is to provide high level clinical pharmacy services and leadership to aid in the care of older persons in the community including those residing in Residential Aged Care Facilities (RACF).

The role will:

  • Work as a member of the Older Persons Emergency Network (OPEN) team to optimise medication management for community dwellers and RACF residents as they receive acute care in their own home.
  • Work as a member of the local Residential Aged Care District Assessment and Referral (RADAR) teams to support medication management for RACF residents.
  • Work collaboratively with the patient and their family, General Practitioner (GP), Community Pharmacy, RACF clinical staff/carers, as well as Hospital Emergency, inpatient and Pharmacy teams to optimise management of patients with complex pharmaceutical care needs in the acute and post-acute care settings.
  • Support models for hospital avoidance and hospital substitution with the aim of providing timely, person-centred care that aligns with the older person's goals of care and best practice.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

  • Provide and be individually accountable for the delivery of high-level clinical pharmacy services specialising in aged care in accordance with prescribed professional and ethical standards. This includes but is not limited to medication history taking, medication reconciliation, conducting comprehensive medication reviews and Therapeutic Drug Monitoring (TDM) with minimal direct supervision.
  • Provide high level advice and assistance to line management, relevant service managers and other members of the multidisciplinary team as required regarding Pharmacy service delivery to diverse stakeholders.
  • Monitor and report on clinical practices and outcomes within clinical service area.
  • Provide education and clinical practice supervision, education and mentoring to less experienced Pharmacist
