An exciting opportunity exists for a Clinical Nurse - Geriatric Evaluation Management and Rehab Services (GEMR), Hospital in the Home (HITH), to co-ordinate and deliver person centred care at an advanced level to all patients admitted to the service. The CN is responsible for supporting the safe clinical management of GEMRHITH inpatients, they lead and promote optimal outcomes for all patients transferring to and being cared for within the GEMRHITH service.

The Sunshine Coast Hospital and Health Service (SCHHS) GEMRHITH is an 8-bed branch of the Hospital in the Home Program providing high quality acute, subacute, ambulatory and community geriatric and rehabilitation service. The service is focused on substituting admitted patient care within the hospital setting to the 'home' setting'. The GEMR HITH  Multi-Disciplinary Team (MDT) service is based at Sunshine Coast University Hospital (SCUH) and provides care to the Sunshine Coast region, operating 365 days a year.

Key Responsibilities include:

  • Adhere to defined service quality standards, health and safety policies and procedures relating to the work being understood to ensure high quality, safe services and workplaces.
  • Fulfil the responsibilities of this role in accordance with Schedule 2, Nurses and Midwives (Queensland Health Award) – State 2015 – Generic Level Statements Nurse Grade 6, Band 1 and the Queensland Public Service values.
  • Understand your broad responsibilities for safety and quality in healthcare, actively participate in continuous quality and safety improvement activities within the workplace and notify a relevant clinical or non-clinical responsible officer when concerns exist about workplace or patient safety.

Remuneration value up to $131,900 p.a., comprising salary between $107,960 - $115,604 p.a. (f/t), comprising salary rates: $54.44 - $58.30p.h. (p/t) employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (NRG6 (1)) (Fixed term temporary full time and/or part time positions available for up to 3 years).
