Become a part of an exciting opportunity as Clinical Nurse Consultant (Clinical Coordinator) with Older Persons Sub Acute and Rehabilitation Service, Integrated Medicine, Emergency, Child and Youth Services. You will apply advanced level clinical leadership and expertise to develop, implement and deliver an innovative and integrated approach to acute and community care for the older person. The Clinical Nurse Consultant will provide professional and clinical leadership across all streams of the service with clinical time to be primarily attributed to the acute stream of the Older Persons Integrated Health Service.

About your role:
- Provides clinical decision making and advice via telephone triage to RACF (Residential Aged Care Facility) clinicians, supported by the Emergency Physician and/or Geriatrician:
- Expert acute / emergency gerontic clinical assessment and care planning skills and use of advanced knowledge of contemporary treatments and outcomes, to facilitate linkage to the most appropriate service to fulfil the resident's acute care need.
- Communicates closely with staff from hospitals, RACFs, GPs, residents, and community groups at the level of expert clinician to facilitate a right care, right place approach.
