About the Role

• Demonstrate comprehensive knowledge of the social factors impacting on health outcomes and incorporates the Principles of Primary Health Care (PHC) in the delivery of clinical care. Displays a proactive commitment to support the acquisition of advanced skills in PHC of self and others.
• Possess experience in collecting and analysing patient and/or service/s data and reporting in achieving key performance indicators and specialist standards.
• Apply change management principles and act as a change agent in the implementation of change strategy within COHS Chronic Disease, Metro South Health@Home and Redland Residential Care programs.
• Demonstrate a high level of confidence and clinical proficiency in conducting advanced, holistic health assessments and providing procedures, treatments and interventions and patient education that is evidence-based and informed by specialist knowledge within the speciality of general medicine and primary care.
