In this role you will be required to provide therapeutic personal care, therapy assistance and other support as directed for Transition Care clients. The successful applicant will provide support to the multidisciplinary team in the promotion of a client focussed service.

About your opportunity:
The position is based at both Cairns North and Gordonvale Hospital and primarily provides services to clients in that geographical area, however; there is an expectation that the position will travel within the Hinterland Hospital Service as required to provide a service to Transition Care clients across Cairns and Hinterland Hospital Health Service.
About your role:
- Provide comprehensive education on a defined range of topics to patients or groups of patients.
- Work across Residential and Community settings of Transition Care Program.
- Lead a defined range of group interventions for patients with diverse and complex needs.
- Actively contribute as a member of a multidisciplinary team, including where appropriate, leading departmental and team meetings, case conferences and other team projects and activities.
- Fulfil the accountabilities of this role in accordance with Queensland Health's core values as well as the department's quality human resource management practices including workplace health and safety, employment equity, anti-discrimination, and ethical behaviour.
