About your role:
- Act as a cultural liaison to advocate, liaise, network and refer to appropriate Non-Government Organisations, Aboriginal and Torres Strait Islander Liaison Officers and the Multidisciplinary Team in collaboration with and under direction of Registered Nurse.
- Provide effective support and advocacy for identified Aboriginal and Torres Strait Islander Rapid Response Falls Service clients by conducting timely home visits, case management/conferencing, follow-up and referrals to relevant health services/programs.
- Coordinate and conduct ongoing health promotion and education sessions with individuals and families within the local Aboriginal and Torres Strait Islander This may include but is not limited to, falls prevention, chronic disease, health screening, and general care for older people.
- Empowering consumers to make informed health care decisions through consultation, education and information sharing.
