An exciting opportunity exists for a Nurse Navigator (Aboriginal and Torres Strait Islander Identified) to join our Community and Preventive Health team. In this advanced practice role, you will provide expert clinical leadership and culturally safe care coordination to patients with complex, chronic, and multiple health conditions. Working in partnership with services such as NCACCH, Cultural Healing, My Aged Care, and NDIS, you’ll support holistic and person-centred care that enhances health literacy and outcomes. This position plays a vital role in improving service integration and supporting culturally appropriate navigation across the healthcare system, while also offering leadership to the Preventable Hospitalisation Program nursing team.

Key Responsibilities include:
- Understand your broad responsibilities for safety and quality in healthcare, actively participate in continuous quality and safety improvement activities within the workplace and notify a relevant clinical or non-clinical responsible officer when concerns exist about workplace or patient safety.
- Practice in accordance with code of ethics, professional standards and legislation affecting nursing practice by maintaining current knowledge and competence within scope of position.
- Provide a central point of communication and engagement to ensure optimal care and coordination of services along a patient's entire healthcare journey are being met – this includes culturally appropriate holistic patient and family centered care. Educate and improve health literacy, to enable patients to self-manage, participate in decisions about their health care, and improve their own health outcomes.
- Adhere to relevant clinical guidelines relating to Aboriginal and Torres Strait Islander consumers (of various ages) and follow ‘close the gap' guidelines on a local and national level to improve systems to enhance overall health and wellbeing.
- Ensure that the Nurse Navigator Program's key performance indicators (KPIs) are met.
- Actively participate in a working environment that supports quality human resource management practices, including employment equity, anti-discrimination, workplace health and safety, and ethical behaviour.
Under section 25 of the Anti-Discrimination Act 1991 (Qld), it is a genuine occupational requirement for the incumbent to be Indigenous to the Aboriginal or Torres Strait Islander Community
Remuneration value up to $13 684 p.m., comprising salary between $5081.10 - $5516.60 p.f., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (Nurse Grade 7) (Fixed Term Temporary position available for 6 months possible extension).
