About the role
The Nurse Practitioner Candidate (NPC) for the Preventative Integrated Care Service (PICS) will be a Registered Nurse who is undertaking a relevant Master's degree to become a Nurse Practitioner (NP), with a focus on Chronic disease management. Nurse Practitioners Candidate are expert, practicing beyond the usual extent of a nurse, at an advanced level, with an expanded scope of practice, and with an advanced degree of autonomy and clinical decision making. They work independently and collaboratively in the health care system.
Fixed Term Temporary Part-time opportunity, until February 2026 (0.7FTE).
About you
Our ideal candidate will be someone who can demonstrate the following:
- Contemporary expert nursing clinical knowledge, expertise and skills in nursing practice and leadership enabling the delivery and supporting high quality care provision in chronic conditions nursing to the level required within mandatory qualifications section.
- The ability to apply specialised knowledge of contemporary advanced nursing practice and theory in chronic disease management and to participate directly and indirectly in the delivery of clinical care.
- The ability to build and maintain strong relationships, open communication and a positive environment within a multidisciplinary team, actively contributing to the learning and development of others to a high standard.
- The ability to educate, inform, counsel and support patients and families with contemporary clinical information at an advanced level relevant to patient's care and understanding.
- Applied knowledge of quality improvement frameworks and associated quality processes inclusive of the integration of nursing research, identifying experiences that evidence a professional commitment to contributing to quality improvement.

Mandatory Requirements:
- Registration with Australian Health Practitioner Regulation Agency (AHPRA) with a current annual practicing certificate is mandatory. Certified copies of the required information must be provided to the appropriate supervisor/manager, prior to the commencement of clinical duties.
- Completed or currently undertaking or has the capacity to undertake the relevant master's program to become an endorsed NP
- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
