What's involved – Working within a community based multi-disciplinary team; the purpose of this position is to: supervise, manage and coordinate a diverse range of Home Care Services available to eligible clients and their carer's. Ensure that home Care Services are planned, implemented, managed and delivered in an efficient and effective manner and coordinate Direct Care Workers in accordance with Commonwealth Home Support Program guidelines and objectives.
