About the opportunity
The Clinical Nurse Consultant – Trauma Program Manager (CNC-TPM), will provide nursing leadership for the development and delivery of the Trauma Connect Project. Trauma Connect aims to provide integrated, efficient, quality trauma care throughout the continuum, and concurrently measure structure, processes, and outcomes in trauma, including post-discharge and long-term outcomes, for the population served by the Major Trauma Service. The CNC-TPM will work closely with the Trauma CNCs to deliver co-ordinated acute trauma care and with the Trauma Nurse Navigator to establish trauma follow-up and outpatient/outreach services and co-ordinated care throughout the continuum, including interhospital transfer and post-discharge.
